Brent Sutter Sports Medicine Clinic Referral
Contact the orthopedic surgeon on-call through RAAPID. The orthopedic surgeon on-call will determine whether or not your patient is appropriate for our clinic. If patient is deemed appropriate by the orthopedic surgeon on-call, fax a referral to Central Alberta Orthopedics at 403-986-7258. You are encouraged to use our Orthopedic Referral Form, and must indicate that this is a Brent Sutter Sports Medicine Clinic referral.
Please ensure the following information is included in either a letter or ambulatory care record accompanying your referral:
- Patient name and contact information
- Name of referring practitioner
- Name of surgeon accepting referral
- Presumptive diagnosis
- Any relevant imaging
If further review of the referral shows it is more suitable for a different clinic, this will be communicated with you. We will contact your patient directly to notify them of their appointment.
