Concussion Clinic Referral
We encourage EARLY referrals and accept referrals from:
- Physicians
- Physiotherapists, Exercise Therapists, Kinesiologists
- Chiropractors, Naturopaths
- Team Coach, Trainers
Fax a referral to Central Alberta Orthopedics at 403-986-7258. You are encouraged to use our Orthopedic Referral Form, and must indicate that this is a Brett Sutter Sports Medicine Clinic referral. Concussion referrals are reviewed in conjunction with Brett Sutter Medicine Clinic referrals.
Please ensure the following information is included in either a letter or ambulatory care record accompanying your referral:
- Patient name and PHN
- Patient contact information
- Name of referring practitioner
- Presumptive diagnosis and relevant clinical history
- Any relevant imaging
If further review of the referral shows it is more suitable for a different clinic, this will be communicated with you. We will contact your patient directly to notify them of their appointment.
