Diabetic Foot Clinic Referral
All patients must be referred by a physician or nurse practitioner To refer a patient, you must contact the orthopedic surgeon on-call through RAAPID. The orthopaedic surgeon on-call will determine if the patient is suitable for our clinic as well as the time frame for consultation.
If your patient is deemed appropriate by the orthopaedic surgeon on-call, fax a referral to Central Alberta Orthopedics at 403-986-7258. You are encouraged to use our Orthopedic Referral Form, and must indicate that this is a Diabetic Foot Clinic referral.
Please ensure the following information is included in either a letter or ambulatory care record accompanying your referral:
- Patient’s name and PHN
- Patient’s contact number
- Presumptive diagnosis
- Name of the referring practitioner
- Name of the orthopedic surgeon accepting the referral
- Time frame for follow-up
- Any relevant imaging studies
Your patient will then be contacted with a date and time for their consultation.
