EMG Diagnostic Services Referral

Currently we are recruiting qualified physicians to come and perform EMG studies in our fully equipped facility at Healthpointe@CAO.  If you are a qualified physician and wish to perform EMG studies in our facility please contact the office manager at 403-309-2001. 

We are temporarily unable to accept referrals for EMG studies until further physician recruitment has been performed.

If you have patients that are suffering from muscle- and nerve-related disorders such as numbness of the hands or feet, weakness, or nerve related pain and wish for them to be assessed in our EMG Diagnostic Services Clinic please complete the EMG Referral Form and fax to Central Alberta Orthopedics at 403-341-6990.

Information to be included in the referral:

• Patient's full name and PHN.
• Current contact information for the patient including phone number and address.
• Name and PRAC ID number for referring physician.
• Office contact information to respond to the referring physician.
• Presumptive diagnosis and brief description of how and when patient was injured to facilitate triaging of the referral.

Your referral will be reviewed and triaged by one of our physicians within 14 days.  We will inform you of the designation and will contact your patient with the appointment time and date.  We will keep you apprised of your patient's progress.
