Ultrasound Guided Injection Services Referral

At Central Alberta Orthopedics, we believe in patient-focused multidisciplinary care.  While we do accept referrals for isolated and targeted injections (for example - RIGHT knee injection with cortisone), we also accept referrals where the type and location of injection is yet to be determined.

As the field of injection medicine expands, the types of injections and where injections can be targeted is growing.  Also, the risk-benefit profile for each injection can be very patient specific.  

We thus offer comprehensive patient assessments and then target the type of injection therapy to each patient combined with a rehab plan and scheduled follow-up if relevant.  Based on the response to injection therapy, we will manage any further patient referrals.

If you would like us to assess your patient and manage the type and location of injection, please fax a completed Musculoskeletal Care Clinic Referral Form to 1-833-627-7022.

Information to be included in the referral:

  • Patient's full name and PHN.
  • Current contact information for the patient including phone number and address.
  • Name and PRAC ID number for referring physician.
  • Office contact information to respond to the referring physician.
  • Presumptive diagnosis and brief description of how and when patient was injured to facilitate triaging of the referral.

Your referral will be reviewed and triaged by one of our physicians within 14 days.  We will inform you of the designation and will contact your patient with the appointment time and date.  We will keep you apprised of your patient's progress.
