Non-Acute Orthopedic Clinics Referral
If you would like one of our orthopedic surgeons to assess and manage your patient's non-acute musculoskeletal injuries, please fax a referral to the Alberta Central Zone Facilitated Access to Specialized Treatment (FAST) program at 1-833-627-7022. This program is a central access and intake program that receives non-urgent surgical referrals for orthopedics and distributes them to appropriate surgical services.
Information to be included in the referral:
- Patient's full name and PHN.
- Current contact information for the patient including phone number and address.
- Name and PRAC ID number for referring physician.
- Office contact information to respond to the referring physician.
- Presumptive diagnosis and brief description of how and when patient was injured to facilitate triaging of the referral.
You may request a specific practitioner, but this may result in longer wait times for your patient.
There is an on-site radiology suite and updated X-rays are not required unless otherwise communicated to you when your referral is triaged.
Referrals will be triaged by an orthopedic surgeon within 14 days and we will notify both you and your patient. We will contact your patient directly to notify them of their appointment.
